New Clients
New clients are always welcome! Whether you are a brand new pet owner coming to a veterinarian for the first time, looking for a second opinion or other options for your long time friend, or have a pet-related emergency, the doctors and staff at Hillside Animal Hospital are here to care for you and your pet. You may print off and fill out the new client form at home using the link below. Filling it out at home may save a few minutes when you arrive at our office or we will be happy to help you fill it out once you arrive for your appointment.
New clients, like all the rest of our clients, are expected to pay for the services provided at the time of service. If you have questions about the costs that may be expected, our receptionist or practice manager can provide that information for most routine services. Our doctors will provide a written treatment plan including diagnostic and treatment costs for all patients requiring hospitalization and for any case at your request before starting treatment. Our practice manager may also be able to arrange special financing options prior to treatment.
When the form opens, please print and complete the information sheet and bring it to the hospital at the time of your appointment.
